Requesting Health Records
You may request a copy of your health record by completing a Request of Information form. Please ask a member of our staff for a copy. A copy can also be found online.

Filing a Consumer Complaint
Report a concern through the Texas complaint process.
To report a concern about care at Family Care, Psychiatry and Behavioral Health Care, or Women's Care Plus, please email: med-patientcomplaint@tamu.edu.
- Report a patient care concern to the Texas Medical Board.
- Report a complaint to the Texas Behavioral Health Executive Council.
